After 17 months of investigation, the National Transportation Safety Board (NTSB) held a hearing, as it works to understand why a door plug blew off a Boeing 737 aircraft over Portland early last year.
A preliminary investigation placed blame on missing bolts meant to hold that door plug in place. That was confirmed, then the board probed further on how it was possible the bolts could be missing in the first place. From workplace and safety control issues at Boeing to failing oversight by the FAA, the board found plenty of blame to go around.
